Chronic Disease Co-Care Scheme
A Government scheme that pairs you with a family doctor to screen for — and then manage — diabetes and high blood pressure, with the cost shared by the Government.

What it is
The scheme works in two stages. First, screening: blood tests to find out whether you have diabetes or high blood pressure without knowing it. Many people do; both conditions are silent for years.
Second, if something is found, ongoing treatment with a named family doctor who follows you over time — consultations, medication, monitoring and complication screening, all subsidised by the Government.
Because you stay with the same doctor, changes get noticed early rather than at a crisis point.
Who it is for
Hong Kong residents in the eligible age range who have not previously been diagnosed with diabetes or high blood pressure. Check the official page for the current criteria, or call and we will check with you.
What we do here
Enrolment in the scheme, screening blood tests taken on site, your results explained in person, and — if a condition is found — long-term subsidised care with Dr Li as your named family doctor.
What to bring
Your Hong Kong Identity Card — eligibility is checked against it.
Your current medicines, and any recent test results or hospital letters.
