What to expect
Chronic conditions are managed over years, not appointments. Seeing the same doctor means changes are noticed — a reading drifting upward, a side effect worth switching away from.
Reviews normally combine a consultation with blood tests taken on site, so treatment decisions are made on current numbers rather than old ones.
Government support for this care
Eligible patients can be managed under the Chronic Disease Co-Care Pilot Scheme (CDCC), which subsidises both the screening that finds these conditions and the ongoing treatment afterwards.
